To all the parents/guardians of our 6th, 7th, and 8th graders. Please read the following information as I hope you will find this helpful.

• All seventh graders and new students to the district are required to have a health appraisal (school physical). A health appraisal done within twelve months of the start of the school year is valid. Please submit a copy by October 16, 2022 to the Health Office.
• All sixth graders (or age 11) are required to have a Tdap vaccine (tetanus toxoid, diphtheria, acellular pertussis) done by October 16, 2022.  A copy of the Tdap vaccine needs to be submitted to the Health Office.
  If your child receives any other immunizations, please send a copy to the Health Office.
• All Seventh graders are required to have the Meningococcal vaccine by October 16, 2022. A copy needs to be sent to the health office. To learn more visit: www.health.ny.gov/immunize
• If your child takes medication in school, parents (or responsible adults) can bring in the medication (in the original container with the pharmacy label) to the nurse along with a parental permission form signed and dated and a doctor's order or script for the medication.

Please call the Middle School Attendance Office ext. 3101 when your child is out for the day. If you do not call, you will receive a message during the day.   Please send in a written excuse the following day with the reason your child was out.
